How to Enroll
1. Complete CAPP Enrollment Form
2. Once we receive the enrollment form, we will schedule a brief meeting with your team at a convenient time, in order to know your practice and team members, and your unique needs. Identifying a clinical practice champion, such as a medical director, or clinician interested in further addressing pediatric mental health, is very helpful in supporting CAPP implementation and utilization.
3. After completion of the enrollment form and agreement, and after reviewing how CAPP works, either through an enrollment meeting, or through our video overview, your practice is considered officially enrolled and can begin benefitting from CAPP consultation, educational events and resources!
